Below are sample Questions and Answers:

HEMODYNAMIC MONITORING- 6-7 QUESTIONS (normal, what to do if its high and
low)
ï‚· Pressure bag- pump pressure bag to 300 and re-inflate PRN, provides continuous
flush so catheter doesn’t get clogged up, pull pigtail qhr to give extra flush
ï‚· Referencing- positioning the transducer at the level of the atria of the heart aka
phlebostatic axis found at 4th ICS mid-axillary (use leveler to get max accuracy)
ï‚· Zeroing out- turn reference stopcock to room air, press the zero button to calibrate
the machine. This makes sure everything is recording accurately. If something is
wrong, re-calibrate and check BP manually
o When to zero- during initial setup, immediately after insertion of line, when
transducer has been disconnected from pressure cable or pressure cable has
been disconnected from monitor (after bathing patient, returning from dx
tests), when accuracy of value is in question. ANY TIME PATIENT MOVES
1.) Pulmonary artery wedge pressure (PAWP)
ï‚· Normal is 6-12
ï‚· Measures LV preload, watch for PA wedge waveform to occur, it will then tell you
the PA wedge reading
ï‚· INCREASED: too much volume from hypervolemia, LV failure, mitral regurgitation
ï‚· DECREASED: hypovolemia
ï‚· Inflating the PAWP balloon: slowly inflate the balloon with air til PA waveform
changes to PAWP waveform. NO MORE THAN 1.5ml of air goes into balloon. Don’t
leave inflated for more than 8-15 seconds. Let the air passively come out, NEVER
pull the air back out.
2.) Central venous pressure (CVP)
ï‚· Assess fluid status, pressure of blood in RA or vena cava. Measured from any
catheter that gives us venous access, commonly done with triple lumen
ï‚· Gives information on intravascular blood volume, RV end diastolic pressure
ï‚· Only looks at RV PRELOAD
ï‚· Normal value is 2-8
ï‚· Complications include infection, air embolus, dysrhythmias, hemo/pneumothorax
ï‚· INCREASED CVP: fluid overload, RV failure, R sided HF, increased blood volume r/t
salt and water, renal failure
ï‚· DECREASED CVP: low fluid, hypovolemic shock, hemorrhage, severe vasodilation
due to septic shock
3.) SVR
ï‚· 800-1200
 INCREASED: vasoconstriction d/t shock, HTN, meds, LV failure  give a vasodilator
 DECREASED: vasodilation d/t reduced afterload, NTG, septic shock  give a
vasopressor
