RE:CorrectiveActions

TO:Allemployees

Effective:Immediately

AsaresultoftherecentinspectionbytheOccupationalHealthandSafetyAdministration (OSHA),severalinfractionshavecometoourattentionthatneedtobeimmediatelyaddressed andcorrected.Itisbecauseoftheseviolationsthatthefollowingcorrectiveactionswillbe

implementedeffectivefromthisdayforward:

1.AlthoughcontrolsareinplaceinaccordancewithCFR1926.353(OSHA,n.d.-d)for properventilationintheweldingshop,basedontheamountofhazepresentduringthewalk- through,thesecontrolsarenotbeingproperlyutilized.Allpersonnelauthorizedadmittanceto theshopshallbetrainedontheuseandmaintenanceofavailableequipmentandwillbe responsibleforensuringuseduringworkinghours.

2.NoweldingoperationswillbeperformednearflammablematerialsinaccordancewithCFR 1626.352(c)(OSHA,n.d.-c),andthepresenceofsuchmaterialsshouldbebroughttothe attentionoftheshopforemanandremovedbeforeweldingisresumed.

3.Employeeswhodisplayfacialhairwillnotutilizetight-fittingrespiratorsinaccordancewith CFR1910.134(OSHA,n.d.-a).Partialsealrespiratorsareavailableandwillbeutilizedbythese individuals.

4.Eyewashstationswillbeaddedtoeveryworkshopthatutilizeinjuriouscorrosivematerials withoutdelayinaccordancewithCFR1910.151(c)(OSHA,n.d.-b)andarerequiredtobe

maintainedandtestedweeklyforcleanlinessandproperoperation.Further,signswillbeposted identifyingthesestations.Inadditiontotheseengineeringcontrols,thefollowingadministrativecontrolswillbe

reviewedforupdatesandadditions:

1.Employeetrainingappearstobelacking.Everyemployeeshouldbetrainedoncompany StandardOperatingProcedures(SOP)priortocommencingwithassignedpositions.Alongwith thecontrolsavailableabove,aswellasthoseforthcoming,PersonalProtectiveEquipment(PPE) incompliancewithOSHAstandardsisavailabletoall.Forthisreason,weeklytoolboxtalkswill commenceeveryMondaybeforestartofbusinessandwillincludeavailableequipment,proper useandfitting,properoperationofcontrolsandequipment,andreviewofSOP.Recordswill includesigninsheetsateverysessiontoverifyattendanceanddocumentacknowledgementof training.Failuretocomplywiththestandingordersandregulationsafteracknowledgingtraining willresultincorrectiveactionsfortheviolatorsuptoterminationforrepeatedoffenses.Failing toproperlyutilizesafetyequipmentandfollowsafetyprotocolwillnotbetolerated.

2.AllshopforemanwillberequiredtoattendOSHA10-hourtrainingonline,andtrainingwill beprovidedtoyoubythecompanyfreeofcost.Youareexpectedtoeducateyoursubordinates onthetrainingyouhavebeenprovidedandcorrectanydiscrepanciesinyourzoneimmediately.

3.SafetyDataSheets(SDS)willbemaintainedforallchemicalsregardlessoflocationina binderandmaintainedbyeachshopforeman.Employee’swillbetrainedonreadingand interpretingthesesheets,haveaccessto,andalwaysknowthelocationofthem.Sheetswillbe reviewedandacknowledgedbysignaturebyeveryemployeenomorethanevery90days,or eachtimeanewsubstancerequiringanSDSisintroducedtothecompany.

4.Thelock-out/tag-outprogramwillbecoveredbytheSOPandindividualprocedure binderswillbeineachzonetobereviewedandsigned-offbyeveryemployeenomorethan every90days.

Thenumberofapparentviolationsnotedintherecentinspectionisunacceptable.Extensioncordsstretchedacrosswalkwayscreateatriphazardandshouldhaveimmediately beenremovedorcoveredandmarked.Unmarkedcontainersshouldhavebeenimmediately recognizedandlabeledassoonasidentifiedorremovedfromthebuildingfordisposal.Ourgoaliszeroinjury,butthiscannotberealizedwithoutzeroviolations.Forthis reason,wewillbepursuingtherequirementstobeacceptedintoOSHA’sVoluntaryProtection Program(VPP)torealizethisresultandensureasafeandqualityworkplaceforouremployees andtheirfamilies.Jerry ccOSHAAreaDirector References UNITEDSTATESDEPARTMENTOFLABOR.(n.d.-a).1910.134-RespiratoryProtection.|
