1. Patient Encounter
- History Taking:
- Patient Name: Lily Johnson
- Age: 4
- Gender: Female
- Chief Complaint: "Lily has been having a fever and a rash for
the past two days."
- History of Present Illness (HPI): Lily's mother reports that
Lily developed a fever two days ago, which has been persistent.
Yesterday, she noticed a red, itchy rash on Lily's chest and back.
Lily has also been more irritable than usual and has a decreased
appetite. She denies any recent travel or known exposure to sick
individuals.
- Past Medical History (PMH): No significant medical history.
- Family History (FH): Mother has asthma. Father has seasonal
allergies.
- Social History (SH): Lily attends daycare and lives with her
parents and older brother. No pets at home.
- Review of Systems (ROS):
- General: Fever, irritability, decreased appetite.
- Skin: Red, itchy rash on chest and back.
- Respiratory: No cough, shortness of breath, or wheezing.
- Gastrointestinal: No vomiting, diarrhea, or abdominal pain.
- Neurological: No seizures, dizziness, or weakness.
