NRNP PRAC 6635 Comprehensive Psychiatric Evaluation Template
Week 8: Training title #114 NRNP/PRAC 6635 Comprehensive Psychiatric Evaluation
Substance abuse and alcohol misuse can lead to and increase a wide range of physical,
psychological, and social problems. It can emerge alongside other vulnerabilities, such as mental
health difficulties, familial, social, or financial difficulties, and experiences of trauma and abuse.
According to the 2018 National Survey on Drug Use and Health, 20.3 million persons aged 12
and up had drug use disorders last year, with alcohol accounting for 14.8 million cases (Chacon
et al., 2021). It not only affects a person's judgment, relationships, or general health and wellbeing. It can cause them to neglect other responsibilities such as school, work, or family.
This assignment aims to thoroughly analyze the patient's condition through a detailed
examination and formulate a diagnosis.
Subjective:
CC (chief complaint): A.C., a 44-year-old female Asian, has been experiencing issues
related to alcohol consumption for an unspecified duration. The problem had become noticeable
enough to prompt intervention from her supervisor and the school EAP counselor. Ally admits to
drinking every night, often alone, “to take the edge off.” She occasionally drinks with friends or
at school functions where alcohol is provided liberally. Acknowledges consuming five or six
glasses of wine or a couple of mixed drinks to feel intoxicated, which is more than the usual
intake of most people. There have been complaints from students and parents about her behavior
in class, including allegations of her sleeping with her head on the desk while students read or
copied notes. She has frequently been late to her classes, with a record of being 22 days so far
this year, and even admits to being late due to drinking too much at a colleague's party and
passing out on the couch. She was charged with DUI when she was 21 years old.
