NURS 225: Nutritional Self-Assessment
Name Tyra Faustino
Part 1: 10 points
Date 9/18/2026
Subjective information. Answer each question:
1. Family History: (check all that apply)
 X_ Diabetes X_ Heart Disease X Cancer
X_ Weight Problems X_ High Cholesterol X Hypertension
Other None
How could these disorders affect your diet?
As a person whose family history consists of diabetes, heart disease, high cholesterol,
and hypertension, I need to eat all foods within moderation. I need to control my sugar
and carbohydrate intake to prevent diabetes. I need to eat low sodium to prevent
hypertension and heart disease. I need to eat low-fat options to prevent high
cholesterol. Overall, I would have to take a preventative measure throughout my life to
prevent getting these disorders since diet is a major role.
2. Culture: How does your ethnic culture or race affect your nutritional choices?
My Filipino culture plays a key role in my nutritional choices. As I was growing up, I ate
rice with meat that contained very little vegetables, fiber, and high amounts of fat and
sodium. Until today, this is my comfort food and what makes me feel satisfied with my
hunger. I rather eat rice and meat than a hamburger or sandwich any day because that
is what makes me feel full.
3. Activity Level:
Never Occasionally X 1-2 times/week 3-4 times/week
5 or more days/week
Describe: I am trying to exercise more consistently, however, as a nursing student and
part-time CAN, I struggle to find the time and energy. I have been trying to go early in the
morning before I begin my study sessions, but then, I feel tired from work or class the day
before.
4. BMI Calculation:
lOMoAR cPSD|42147428
63 inches Height 170 lbs Weight 30 BMI
