Reason for Encounter
A 33-year-old female presents to the outpatient clinic with complaints of
right-sided flank pain for the past 3 days.
Chief Complaint (CC):
"Right-sided flank pain for the past 3 days."
History of Present Illness (HPI)
The patient reports experiencing a dull, aching pain in the right flank that
started 3 days ago. The pain is constant, rated 6/10 in intensity, and has
progressively worsened over time. There are occasional sharp exacerbations of
pain that make her feel nauseous, but she denies vomiting. She describes the
pain as deep and localized to the right flank without radiation to the back,
abdomen, or groin.
The patient has not had any trauma to the area, and she denies any recent
changes in physical activity or diet. She reports increased urinary frequency
and urgency over the past week but denies dysuria, hematuria, or foul-smelling
urine. She has no fever, chills, or weight loss. The patient has a history of
recurrent urinary tract infections (UTIs), with her last episode occurring about
a year ago.
