Answer: A list of all medications that a patientis taking whether it is prescribed by a physician
or purchased OTCs as well as known drug allergies, height, weight, medical conditions,
discontinued medicationsand medications tried in the past
2. Medication Reconciliation
Answer: The process of identifying the most accurate list ofall medications that the patient is
taking including name, dosage, frequency and route by comparing the medical record to an
external list of medications obtained from patient, hospital or provider
3. Medication Adherence
Answer: The extent to which patients take medication as pre-scribed by their doctors such as
getting prescriptions filled, remembering to takemedication on time and understanding the
directions
4. Primary Non-Adherence (PNA)
Answer: Patient takes the time to go to the physician toget diagnosed and does not get the
prescription filled
5. Secondary Non-Adherence (SNA)
Answer: Patient took the time to visit the physician and picked up the prescription from the
pharmacy then started using the medicine but didn't take it as prescribed (i.e. insufficient
doses required to experience therapeutic effect, missing doses, discontinuing therapy early and
medication possessionrate over 80%)

Answer: identify patientsthat need assistancewith using drug therapies correctly; prevent
