How do you manage gestational diabetes?

Answer:

Diet control, exercise, record log of glucose readings. How to recognize hypoglycemia (shaky, dizzy, weak)

If not working you will treat with insulin - the oral hyperglycemic you would use is Glucophage

Do you check blood sugar after delivery for mom?

Answer:

Yes

Continue insulin meds you will taper them off 1 / 4

Placenta Abruption & the 2 types

Answer:

Placenta separates from the uterine wall before delivery... very painful!

Concealed: only small amount separates around edges and the blood will pool

behind and wont come out of vagina

Complete: placenta isn't attached to uterus anymore and will see vaginal

bleeding

Question:

What do we do for placental abruption?

Answer:

Small amounts of vaginal bleeding: if the fetus is stable and has good

variability and accelerations we will monitor Large amounts of vaginal bleeding and the fetus is in serious distress with late deceleration they will have to deliver

First action is to put on a fetal monitor and assess baby

Answer:

Painful Sometimes vaginal bleeding (dark blood) Uterus irritability

Question:

Placenta Previa & the 3 types

Answer:

Placenta implants too low in the uterus and it is over the cervical opening

Bright red painless bleeding

Complete - you can't see cervix and must do c section Partial - you might be able to see cervix but still a c section Marginal - off to the side and you can typically deliver vaginally

Answer:

Screening:

Ultrasound, alpha fetal protein, 1 hour glucose challenge test

Diagnostic:

Amniocentesis, 3 hour glucose tolerance test, Chorionic Villus Sampling

Only time ultrasound is diagnostic is the first one to confirm pregnancy

Question:

Risks of gestational diabetes

Answer:

Shoulder dystocia, metabolic disturbances, preeclampsia, polyhydramnios and fetal macrosomia
