Scenario: Larry, a 64-year-old male, presents to his primary care office with complaints of a painful, swollen, right big toe. The pain began suddenly 2 days ago and has been getting progressively worse. The client denies any recent accident or injury. The pain is constant but he feels it’s worse with weigh bearing. He currently rates his pain as a 7/10 and describes the pain as “throbbing.” He finally decided to come into the office today because even the feel of the bed sheets against his toe was awakening him from sleep last night. His past medical history is significant for hypertension and obesity. He was prescribed hydrochlorothiazide, a diuretic, last month to help control his blood pressure and started a low-carb diet in an attempt to lose weight.As a result, his current diet has included more animal protein over the past month specifically
turkey, beef, and chicken. Objective examination of the client reveals:
Swollen, erythematous right, great toe Temperature 98.4°F, temporally Warmth with palpation right, great toe +1 nonpitting, pretibial edema in the bilateral lower extremities +2, regular pulses of the dorsalis pedis pulses No evidence of rash Negative inguinal lymphadenopathy Firm, pea-sized nodule on the right, great toe Blood pressure 128/76 mmHg Full active and passive ROM of the right foot and ankle
