Ventricular tachycardia

must determine if STABLE or unstable before deciding anything!!

1st: is there a pulse?--> YES--> is it stable or unstable?

UNSTABLE VT= SIGNS of instability: hypotension, alt

mental, shock, chest pain, acute heart failure --> synchronize cardioversion STABLE V T= no se of instability? --> give them an arrhythmic medication (amioderone/lidocaine) NO PULSE --> START CPR AND PREPARE TO DEFIB *must know by sight!* Accelerated Idioventricular Idioventricular Torsades de Pointe AM!!caused by PROLONGED QT interval usually due to HYPOMAGNESIUM or meds

TX:

  • IV Magnesium
  • defibrillation
  • d/c qt-prolonging meds
  • rash with honey colored crustimpetigo very contagious infection who can care for a pt receiving chemo??- MED SURG NURSE CAN

  • pregnant nurse (wont be administering chemo and chemi is not radioactive so
  • preg pt is good)

  • nurse who got flu vaccine!

CHF patients s/sright sided (systemic):

JVD, elevated CVP, dependent pitting edema of extremities

left sided (pulmonary): CRACKLES
